Behavior Plan
For use by school personnel and parents to monitior, motivate and help student to become self aware of behaviors that interfere with learning and to develop identified skills.
Release of Information
Gives legal permission for school personnel to share confidential information with authorized staff from another agency or school.
Banana Splits Group permission form
Guardian/parent permission for student to participate in a support group addressing issues related to families where the parent/guardians are no longer living together. The Banana Splits program was developed by Elizabeth McGonagle.
Social History Guidelines
Social history information is gathered by the social worker, nurse, guidance counselor, or psychologist and summarized for the committee on special education when a student is evaluated for a possible disability that may be an obstacle to academic success. These are possible questions that are asked of the parent/caretaker.
Pathways Inventory
This is a skill inventory form to be used with a Functional Behavior Plan. It was developed by Ross W. Greene Ph.D. who wrote, The Explosive Child. For more information, visit the Center for Collaborative Problem Solving website.
