New York State Public Health Law, Section 2164, requires all school children be adequately immunized. No child may be allowed to attend school without proof of adequate immunization. New York State Law requires physician verification of DATES for the following immunizations for legal entry into any New York State school.

Your child must comply with the following requirements to attend the first day of school:

• DIPHTHERIA (DPT or DT) 3 DOSES
• POLIO 3 OPV or EIPV
• MEASLES Pre-K: 1 dose of Measles, Mumps, Rubella (MMR)
• K-12: Born before 1985: 1 dose of MMR
  K-12: Born on or after 1/1/85 2 doses of measles
  Containing vaccine and 1 dose each of
  mumps and rubella (preferably as MMR)
• HEPATITIS B Pre-K: Born on or after 1/1/95: 3 doses
  K-12: Born on or after 1/1/93: 3 doses
  And Grades 7-9
• HIB Pre-K: 3 Doses if less than 15 months of age OR
  1 dose on or after 15 months of age
• VARICELLA Pre-K: Born on or after 1/1/2000: 1 dose
  K-12: Born on or after 1/1/98 1 dose
  6th gr: Born on or after 1/1/94 1 dose
• TETANUS DIPHTHERIA PERTUSSIS (Tdap)
  6th gr: At age 11 years old

EXCEPTIONS

1. A certificate signed by a physician licensed in New York State stating the specific reason or condition why immunization(s) are detrimental to the child's health.

2. A Written statement signed by the child's parent/guardian that they hold religious beliefs contrary to the practice of immunization. The statement must describe the beliefs in sufficient detail to permit the school to determine that (a) the beliefs are religious in nature (not health or philosophical), and (b) the beliefs are sincerely and genuinely held.
